Oncology referral process

It is mandatory to follow the steps below to enable your patient to be seen for a consultation with a fertility specialist within 24 hours (72 hours if referred on a Friday).

This will allow the patient’s cancer treatment to commence with as little disruption as possible by ensuring that their fertility preservation is completed effectively. 

• All fields on the form below must be complete and include confirmation of rapid authorisation from the CCG funding treatment otherwise we will be unable to accept the referral for fast track.
• Patients must have serology results sent with the referral – Hepatitis B Surface Antigen (HBsAG), Hepatitis B Core Antibody (Anti-HBc), Hepatitis C Antibody (Anti-HCV) & HIV 1 & 2. This is a strict requirement by the HFEA. 
• Female patients BMI must be below <35 due to the increased risks of intravenous sedation.
